Photo Use Consent

By uploading photos of my pet, I understand and agree that:

• Holly Ridge Veterinary Hospital may store my pet’s photo in their medical record.
• My pet’s photo may be shared on social media or other marketing materials if I indicate “Yes” below.
• If I give permission for social media use, I grant Holly Ridge Veterinary Hospital a non-exclusive, royalty-free right to use, edit, and share my pet’s image, first name, and general information (for example, “Bella the golden retriever”) on its website, social media accounts, and other promotional materials.
• I am the owner or authorized agent of the owner of the pet in these photos and have the authority to grant this permission.
• I understand that I may contact Holly Ridge Veterinary Hospital at any time if I would like to withdraw permission for future use of my pet’s image on social media or in marketing materials. This will not affect photos already used prior to my request.
